Metformin and Ozempic are both approved to improve glycemic control in type 2 diabetes, and there the similarity ends. One is a generic tablet of a molecule in use for decades, the other a once-weekly injection of a GLP-1 receptor agonist. Even their boxed warnings are about entirely different risks. Ozempic injection
  • Administer once weekly at any time of day, with or without meals.
  • Start at 0.25 mg once weekly. After 4 weeks, increase the dosage to 0.5 mg once weekly.
  • If additional glycemic control is needed, increase the dosage to 1 mg once weekly after at least 4 weeks on the 0.5 mg dose.
  • To reduce the risk of sustained eGFR decline, end-stage kidney disease and cardiovascular death, increase the dosage to 1 mg once weekly after at least 4 weeks on the 0.5 mg dosage.
  • If a dose is missed, administer within 5 days of missed dose.

Warnings and precautionsMetformin hydrochloride
  • Lactic Acidosis: See boxed warning.
  • Vitamin B 12 Deficiency: Metformin may lower vitamin B 12 levels. Measure hematological parameters annually and vitamin B 12 at 2 to 3 year intervals and manage any abnormalities.
  • Hypoglycemia with Concomitant Use with Insulin and Insulin Secretagogues: Increased risk of hypoglycemia when used in combination with insulin and/or an insulin secretagogue. Lower dose of insulin or insulin secretagogue may be required
Ozempic injection
  • Acute Pancreatitis: Has been observed in patients treated with GLP-1 receptor agonists, including OZEMPIC. Discontinue if pancreatitis is suspected.
  • Diabetic Retinopathy Complications: Has been reported in a clinical trial. Patients with a history of diabetic retinopathy should be monitored.
  • Never share an OZEMPIC pen between patients, even if the needle is changed.
  • Hypoglycemia: Concomitant use with an insulin secretagogue or insulin may increase the risk of hypoglycemia, including severe hypoglycemia. Reducing dose of insulin secretagogue or insulin may be necessary.
  • Acute Kidney Injury Due to Volume Depletion: Monitor renal function in patients reporting adverse reactions that could lead to volume depletion.
  • Severe Gastrointestinal Adverse Reactions: Use has been associated with gastrointestinal adverse reactions, sometimes severe. OZEMPIC is not recommended in patients with severe gastroparesis.
  • Hypersensitivity Reactions: Serious hypersensitivity reactions (e.g., anaphylaxis and angioedema) have been reported. Discontinue OZEMPIC if suspected and promptly seek medical advice.
  • Acute Gallbladder Disease: If cholelithiasis or cholecystitis are suspected, gallbladder studies are indicated.
ContraindicationsMetformin hydrochloride
  • Metformin hydrochloride tablets are contraindicated in patients with: Severe renal impairment (eGFR below 30 mL/min/1.73 m 2).
  • Hypersensitivity to metformin.
  • Acute or chronic metabolic acidosis, including diabetic ketoacidosis, with or without coma.
  • Severe renal impairment (eGFR below 30 mL/min/1.73 m 2)
Ozempic injection
  • Personal or family history of MTC or in patients with MEN 2.
  • Serious hypersensitivity reaction to semaglutide or any of the excipients in OZEMPIC.
Drug interactionsMetformin hydrochloride
  • Table 3 presents clinically significant drug interactions with metformin hydrochloride.
  • Carbonic anhydrase inhibitors may increase risk of lactic acidosis.
  • Consider more frequent monitoring
  • Drugs that reduce metformin clearance (such as ranolazine, vandetanib, dolutegravir, and cimetidine) may increase the accumulation of metformin.
  • Consider the benefits and risks of concomitant use
  • Alcohol can potentiate the effect of metformin on lactate metabolism.
Ozempic injection
  • Oral Medications: OZEMPIC delays gastric emptying.
  • May impact absorption of concomitantly administered oral medications.

One disease, two very different documents

The metformin label describes a biguanide indicated as an adjunct to diet and exercise to improve glycemic control in adults and pediatric patients 10 years of age and older with type 2 diabetes.

The Ozempic label covers glycemic control in adults, and two further indications metformin's label does not have: reducing the risk of major adverse cardiovascular events in adults with established cardiovascular disease, and reducing the risk of kidney function decline, end-stage kidney disease and cardiovascular death in adults with chronic kidney disease.

The metformin label reaches younger patients; the Ozempic label reaches further into the complications of the disease.

The boxed warnings do not even overlap

Metformin's boxed warning is about lactic acidosis, a rare metabolic complication whose risk factors the label lists at length. Ozempic's boxed warning is about thyroid C-cell tumors observed in rodents, the warning shared by the GLP-1 receptor agonist class.

This is unusual for two products treating one disease, and it is a reminder that the risks named by the regulator follow the molecule, not the diagnosis.

A tablet regimen against a weekly injection

Metformin is taken orally with meals, starting at 500 mg twice a day or 850 mg once a day, increased gradually to a maximum of 2550 mg per day in divided doses. Ozempic is injected subcutaneously once weekly, starting at 0.25 mg with stepwise escalation.

Are metformin and Ozempic in the same drug class?
No. The metformin label describes it as a biguanide. The Ozempic label describes semaglutide as a GLP-1 receptor agonist. They are different classes with different mechanisms and different boxed warnings.
Is metformin approved for weight loss?
No. The metformin label carries one indication: improving glycemic control in type 2 diabetes. It appears in this category because it is often discussed alongside newer medications, not because of its approved use.
Why is there a generic metformin but no generic Ozempic?
Metformin has been approved long enough for generic applications to be filed and approved; the FDA label database lists dozens of them. The semaglutide products hold newer approvals, and no generic application for them appears in the database.
